Description of presentation


Health care reform is anything but static. Adapting to and leveraging new payment models and an array of proposed quality of care measures constitute the most significant strategic challenge for virtually every sector in Minnesota’s health care system. This presentation will highlight several health care reforms and the incentives, pressures, and opportunities that are just around the corner to include the expected proliferation of measures and payments based on episodes of care, as well as new provider incentives under the Medicare Access and CHIP Reauthorization Act (MACRA).

  1.  Understand the increasing strategic importance of cross-continuum collaboration and managing financial risks in light of new episodes of care payment models.

  2. Understand how new payment models will correspond to healthcare workforce recruitment and retention.

  •   David W. Moen, MD


            Dr. Moen has spent his career providing thought leadership to teams achieving nationally-recognized and market-changing Triple Aim results for high risk patients in hospital, clinic, and community settings.


 


Description of Presentation


Dr. Moen will be discussing emerging care delivery models that better manage high risk patients and transform the experiences and outcomes of patients with chronic disease.  These models have demonstrated effectiveness and efficiency but still face real barriers with regard to implementation in the marketplace.  


Learning Objectives



  1. Understand the operational characteristics and effectiveness of care models designed to serve frail elders.

  2. Define current trends in disease management and new treatment approaches that can improve effectiveness and efficiency. 

  3. Discuss the barriers that innovative care models face in today’s market.
